Survey on Gr.1 to 6 students' willingness of take flu shot

親愛的家長，您好！

    最近天氣多變，也讓「流感」再度成為孩子健康的隱憂。

    學校於10月16日為國小一到六年級學童施打流感疫苗，但仍有許多學生沒有接種流感疫苗，為有效預防流感及流感的傳染，新竹市政府將提供第二次的預防接種，如果您的孩子尚未接種流感疫苗，但目前有意要讓孩子接種流感疫苗，請您與雙語部衛生組聯繫。   

為節省您的時間與花費，如果全校國小學生合計超過30人有意接受流感疫苗接種，新竹市衛生局將會派人至學校辦理流感疫苗的接種工作。

Dear Gr.1 to 6 parents,

Recently, the weather changes a lot.  Our kids are now facing the threat of the influenza again.

On October 16, most of elementary students took the influenza vaccine.  However, still some of the students did not take the influenza vaccine.  In order to keep elementary school students away from the threat of influenza, the health center once again surveys the students about their willingness to take the influenza vaccine.   To minimize the time and cost of parents having to take students to the health centers or clinics to get the influenza vaccine, the Hsinchu City Government will send a medical team to school to give students the flu shot, if there are more than 30 students willing to take the flu shot.

If you want your kid to take the vaccine, please click the following link and fill out the form and the sanitation section will contact you as soon as possible.  The survey will end at 11:00 a.m. on Friday, December 7.  If you have any questions about it, please contact Ms. Savannah.
